Medical Paperwork Your Scout Must Complete


To comply with BSA Regulations and to enable the leaders to keep their sanity, each Scout is required to have completed medical and up-to-date contact information on file for each and every outing. That's right, the Scout Master needs all the applicable forms on file before we can even go down the street to get ice cream! Our rules are quite simple: No medical forms equals No Outing. Period. No exceptions, unless accompanied by a parent.

Starting in 2010, BSA Medical forms have been greatly simplified. You may open the BSA form #34605 and fill it in on-line and then print it, or save it to your hard drive. A doctor's signature is required. The rules are:


  • All Scouts must have the completed BSA #34605 medical form signed by a doctor of medicine (MD).

  • All Adults must have the completed BSA #34605 medical form signed by a doctor of medicine (MD).

  • All Scouts must have the completed Troop 518 Waiver.


    Remember:

  • It is the parent's responsibility to supply new copies with the latest Scout and family information on or before June 1.

  • Do NOT bring us the originals. We only want photocopies.

  • Due to inclement weather during Scout outings, leaders are not responsible for keeping your medical forms in perfect condition. The adult leadership may ask for new copies of your forms at any time. Again, parents keep the originals.
